ROLE AND RESPONSIBILITIES
The primary responsibilities of the Community Health Volunteer (CHV) will include conducting patient intake procedures, identifying and understanding relevant social determinants of health, and connecting patients/clients with appropriate community resources. The CHV will accurately document social and other relevant information in the appropriate online databases and assess whether patients require additional support or follow-up services .Specific responsibilities include:
- Recording of patient demographic information into Athena EMR and/or the CM99 database
- Assisting with patient vitals/intake
- Knowledge of community/social work programming and resources for specific patient cases
- Handling and dealing with sensitive/personal patient information
QUALIFICATIONS AND EDUCATION REQUIREMENTS
Community Health Worker Experience/Program Enrollment
~Must be a committed clinic volunteer willing to help out at least 2x/month for 6 months
